Yes, that is the much under-rated "French" Bash. Discussed at length back in '98 on the old site.
Oddly enough, Holt wears all his Beret's with that exact same bash.

You can see the bag on his left side and the part of the strap (going the other way), where the horses ear is (looking at the photo)
and Indy is not left handed (holding the gun)!

The dent might well have been made by Ford while trying to stop the hat flying off his head...As we've all seen in the extras section, he found a better method in the end, but perhaps this was a "quick grab" moment to stop it flipping off....It looks like a left handed finger pattern to me...
As for Ford and the horse levitating...Don't be silly...Ford is a human, he can't levitate...It's just the horse..("Shadowfax" I believe?)

When a horse is running at a full gallop it does have all four hooves off the ground at times.

I bashed one of my fed's just like that....
....Woke up in the hospital a few hours later! (I kid...I kid....)
